The GWPCA Board has formed a Canine Health Information Center committee to be able to decide what tests based on a survey from the GWPCA members will be required in order for a GWP to be issued a CHIC number.

The answers to the questions on this form will determine what tests will be required. If you would like more information on the CHIC registry, please visit their website at www.caninehealthinfo.org We have listed current available statistics below for GWPs per the Orthopedic Foundation for Animals for hip and elbow dysplasia, thyroid and cardiac.
